Activity Overview


When setting goals or reflecting on their growth, it's helpful for students to have something to refer back to where they've recorded a moment in time. This worksheet is designed to give students a space to actually write down some of the information they identified in other activities in this lesson plan (although it can be used independently), and set goals for themselves. Set aside time for students to check in with how they're doing after three or six months, and see if they need to change anything else. If needed, adjust the time span for your unit plans or school year, but students should check in with their goals at least once during the year.

Student Instructions

  2. Identify the top 3 areas of wellness that you would like to boost.
  3. In the first box in each row, describe these 3 areas and what changes you will make to improve and maintain them.
  4. Check in and write notes after three months, and again after six months.
  5. Save and Exit
