- Over confidence has been blamed for the sinking of the Titanic, the nuclear accident at Chernobyl, the loss of the Space Shuttle Challenger and Columbia, and the Deepwater Horizon oil spill.

- Identifying overconfidence early in the process of completing a task will increase the likelihood that the task will be completed with error. High risk situations require diligence in identifying the overconfidence error precursor either by oneself or by the peers they are working with. Remember, be confident but do not allow yourself to become OVERCONFIDENT.
