- Greg recited his drug study of piperacillin in front of his clinical instructor.
- Ma'am. Ang medication po ng patient ko ay Piperacillin, po. Antibiotic po ito. Given IV sa patient, po. Since every 6 hrs po ito at ang last dose ni patient kay 7 am, oras na ibigay sa kanya ang gamot dahil 1 pm na, po.
- 1:00 PM
- Instruct the patient to report any signs of adverse reactions such as pruritus and/or erythema.
- Greg presented his FDAR to his clinical instructor for evaluation.
- 1:30 PM
- Follow the doctor's order, ha? Huwag itong gawin na independent nursing action unless indicated.
- Ang focus ko po is impaired gas exchange related to pulmonary obstruction.I administered oxygen therapy 5 L/min, as indicated at 8:25 am, po.

- After final VS plotting and endorsement of the patient's I&O to the nurse on duty, Greg bid farewell to his patient at the end of his shift.
