Macbeth isn't sure whether the dagger before him is real or not as his mind is so confused that the line between real and imaginary is becoming blurred.
It is now time! Once he has killed Duncan, Macbeth will be king and fulfil the prophecy!

He knows what he has to do to fulfill the witches' prophecy and satisfy his wife, and the dagger is leading him the direction he must go. More blood from the dagger appears and Macbeth becomes more anguished.
