Hmm...I wonder what's going on in the lunch room. It's too far away for me to see.
Uh-oh! I just crashed hard into that roof support pole.

I’ve met your kids and I want you to always go home safely to them. Please remember your training and always be aware of your surroundings when operating a forklift.
Are you okay?
I'm sorry, Safety Steve. I wasn't paying cllose enough attention.
S
I'm not hurt, just a little embarrassed. I should know better than to look away when on the forklift.
I'm glad you're okay. I’ve seen you driving before without looking where you’re going and I should have said something to you so this wouldn't have happened.
S
Lisa, you should always feel comfortable speaking up. Let's make some changes around here so this doesn't happen again.
The next day the team painted the warehouse beams bright yellow and put up column protectors. They held a team meeting talking about the incident and encouraging people to communicate with each other about potential hazards. They reminded everyone that all of them have a right/duty to stop work when they see an unsafe condition.
S
This will help make sure everyone sees the poles so they aren't as much of a hazard.
