- The manager assigns a task that has several steps. They offer the chance to ask questions. Despite the fact the employee is unsure of one of the steps, they do not ask and decide they will figure it out when they are working on the task. They don't want to seem like they are unsure or are not good at the job.
- The employee knows how to perform this step of the task and begins properly while adhering to proper protocols by using safety lockouts, turning off power to a machine and checking it is ready for use.

- The employee needs a part from a high location. The part is heavy and they should use a ladder. They don't find a ladder after a quick look, and decide to use a chair to reach, they don't want to go to their manager about a ladder.
