This storyboard describes the journey of a patient with suspected stroke presenting to the hospital, from admission to discharge.

- Paramedics convey patient with stroke symptoms to ED. Patient is within 4.5 hour thrombolysis time. Staff nurse meets paramedic crew at front door and confirms stroke team had been activated. Patient is taken to the resuscitation room

- I am administering the thrombolytic
- Stroke physician and stroke nurse evaluate patient. Patient has stroke symptoms and the arrange CT brain.

- I have completed the occupational therapy plan
- I have completed the physiotherapy plan
- CT brain confirms acute ischaemic stroke. Radiologists conveys this information to stroke physician. Patient will need thrombolysis.

- Thrombolysis medication administered
- Tom is transferred onto the stroke unit where he receives multidisciplinary care with rehabilitation included. He responds well and does not need to go to a rehabilitation unit after discharge.
- I have completed the dietary plan
- Tom has been discharged home and his family is there for support.
