Paramedics convey patient with stroke symptoms to ED. Patient is within 4.5 hour thrombolysis time. Staff nurse meets paramedic crew at front door and confirms stroke team had been activated. Patient is taken to the resuscitation room

Tom is transferred onto the stroke unit where he receives multidisciplinary care with rehabilitation included. He responds well and does not need to go to a rehabilitation unit after discharge.
I have completed the dietary plan
Tom has been discharged home and his family is there for support.
