  • Jimmy, Mrs. Vanderbilt can go home but she needs a PICC Line Placed first for her Antibiotics. I just entered the PICC consult order.

  • 1000 - Good to go but....
  • Rm. 214
  • OK, Dr. Sanders, Let me coordinate that with the Care Team
  • 11:30 Everyone's notified, right?
  • OK, I think I've called everyone, Jake the Nurse, Jan the D/C Planner, Karrie with PICC, and Beth from X-Ray should all be aware
  • The PICC team came by and inserted the line but we'll still waiting for X-ray for Mrs. Vanderbilt
  • 1500 - What??
  • What? I thought everything would be done by 1300 and Mrs. Vanderbilt would be discharged. This is poor coordination!
  • After

  • Ok Dr. Sanders, I'll Iris the Care Team and the PICC Team so that they all know this is the only item we're waiting on. I'll also assign myself a follow up task in CA.
  • Trying to get everyone on the same page with the patient's progression needs.
  • 11:30 Time to follow up
  • I can see in the E.H.R. Notes that the PICC is placed but we're still pending X-ray. Let me Iris them to make sure they're on it.
  • Hey Jimmy, Mrs. Vanderbilt just left and her room is clean. Do you have another patient for me?
  • It's hard to stay on top of an activity with this many moving parts. Jake tells Jimmy that the PICC is done but X-ray hasn't come by yet.
  • 1400 - What's next?
  • I sure do. Bed placement just called looking for a monitored bed and I told them 214 is ready.
  • Everyone on the care team as well as ancillary services are notified immediately of the pending discharge with contingencies.
  • It's so easy to check the To-Do list for each patient and follow up.
  • Patients are progressing as they should due to communication and coordination enabled by Care Logistics
