I think that Dr Mortimer is a successful elderly medical man who is well-esteemed.
Holmes and Watson finding a conclusion to who Dr James Mortimer is by his stick.
I feel he is a grave, middle-aged family practitioner

Holmes introduces Watson and himself to Dr James Mortimer who just entered the house.
