  • Good morning Ma'am, I am Dexie, your nurse for today. May I know your name and birthday? Also, could you tell me why you're here?
  • I am Rosalie Nunez. November 17, 1972. I am here because I have been experiencing chest pain.
  • May I know what makes the symptoms better or worse? Where is the pain located? From 0 to 10, How bad is it at its worst? When or at what time the pain started? How long did it last? How often does it occur: hourly? daily? weekly? monthly? Is it sudden or gradual? What were you doing when you first experienced it? When do you usually experience it: daytime? night? early morning? Are you ever awakened by it? Does it lead to anything else? Do other signs and symptoms accompany it?
  • I am not sure but I think, It was last 2 weeks, June 25, at the afternoon. It feels like pressure, tightness, pain, squeezing or aching. I also experience discomfort that spreads to the left shoulder, left arm, back, neck, jaw, teeth or sometimes the upper belly, and light headedness. It's Intense and unremitting for more that 15 minutes and it's 9-10 in scale. I was taking a jog when it happens at first and it happens again when I do some exertion activities
