Total wellness is the total care of your body socially, physically, and mentally/emotionally. Without proper care, it can lead to a dissatisfied life, along with possible health issues.

Social wellness is how well you can do in society and your ability to adapt to a social conflict. This could be your healthy relationships with family and friends. If something should arise, you should be able to resolve with minimal damage.
How to Improve on the Three States of Wellness
Physical wellness is proper care for your body. Some examples include working out, proper hygiene, etc. Without proper care, however, some of your body's functions will start to deteriorate and won't work well as it should.
Mental/emotional wellness means how well you are thinking, feeling, and acting. Examples are feelings of happiness, sadness, anger, stress, confused thoughts, poor behavior, etc. If not taken care of, it can lead to illness and possibly death.
Some ways to improve your social wellness are interacting with others or seeking professional help. A physical wellness care could be a possible diet, less intensive workouts if it is too much for your body to handle. A mental/emotional help would be to free up your schedule to lessen the stress, ask advice from a friend or professional, etc.
