- Hi, I', Mavins, social care have provided a good care package for me when I leave hospital but the discharge from hospital has been badly handled and I haven't been consulted on the care being put in place for me..I am dissatisfied with the amount of choice and control over the support provided, with the amount of information to make decisions, and with the lack of support to manage my health/care.
- The hospital discharge process wasn't a good experience for me, decisions were made for me not with me. I wasn't consulted or given any choice about my care arrangements for when I got home; no one told me what was going on even when moving me to the discharge ward waiting to go home. I should have been kept informed about what was happening, when and why; As a patient I was as not treated with respect and dignity. I don't believe that discharge outcomes were not met. I wasn't given any information that could have influenced better outcomes for myself. My carers aren't reliable, they don't work together for me to achieve what I want to during the time they care for me. There is a lack of common sense; they don't help me with specific things.
- An ideal discharge would have started with including Mavis and her family in discussions about her discharge from hospital and the care she would need when she got home.
