- My Name is Richard, I have a long term health condition and I am a little forgetful now days and rely on my Wife Jean to understand what is going on and reassure me, along with making sure my views are heard.
- My Name is Jean, whilst Richard has been in hospital, the staff have kept us informed of Richards progress and have provided us with reassurance when required.
- Richard is doing very well and should be ready to go home tomorrow. We will work with you to plan Richard's discharge andensure you have the support in place for Richard to return home.
- Thank you for yoursupport, and keeping us well informed, it has relieved our fears and we are looking forward to getting home,
- Richard is ready for discharge and as discussed, we have referred Richard for some short term care, called reablement, to continue Richards recovery at home and enable Richard to become as independent as possible. We have also arranged for some equipment to be put in place to assist you, and for some physiotherapy. Here is our discharge leaflet.
- I am Sally, Richards Social Worker, I will continue to support you both once Richard is home.
- Richard has nearly achieved all of the goals set and so we will be looking to end the reablement service and have sent views of others to social care to source an ongoing care provider and for Physio to continue for a bit longer.
- Our Social Worker called today and is coming to complete a review next week and has said that she has made a referral for ongoing care and physio, she has also offered me a carers assessment.
- The whole process has been seamless and everyone has taken the time to explain what is happening in detail so that both I and Richard are aware and our views have also been listened to .
