1 or 2 pages containing details about the general FTP process including any inforgraphics or explanatory videos; metrics about concerns received and other FTP data such as the hearings calendar; links to other GDC documents/policies/processes. Initial information about sharing a concern or reporting an individual/premises and a link to the portal's landing page
This page will ask questions to start the user on the appropriate route including: have you already raised a concern/do you have an existing concern reference & are you a patient/member of the public or are you a pharmacy professional/raising a concern on behalf of a registered organisation/company
Complain about the GPhC, a service provided by or function of the GPhC, or the outcome of a Fitness to Practice case
This page will outline what the GPhC will do, won't do and can't do. Before progressing the user will tick a box to confirm they have read this information. There will also be links to content (if necessary) that details more explicitly the GPhC's remit and provides further on this depending on the page design. There will also be links to content if the user realises that the concern is not for the GPhC such as a patients' help section which could contain more information about who to complain to, and contact details for support services who can help.
I've read the above and am sure my concern is for the GPhC
This page will get the PRC to confirm their willingness to work with the GPhC. If they say no it will link them to information/another page that identifies the investigative limitations of not consenting and give them the option of still continuing with their concern. The whistleblowing link will take the user to more information and a link to the whistleblowing policy/email address with the option of coming back here instead The green link will move them to a modified concerns form/intelligence record
This page identifies what the GPhC needs from the person raising a concern It will also identify the types of evidence that might be required in different cases and how the PRC should go about obtaining/securing this evidence in advance of raising the concern. It should also offer a link to the PRC at this stage to not raise a concern but to provide us with intelligence in the alternative and link to the intelligence record
On looking at it again I think I want to share my experience/tell you something but not raise a concern that requires my continued involvement
We can decide whether there should be multiple concerns forms depending on the preceding choices or whether depending on those choices certain fields will be mandatory or not. If we haven't included it previously there will be a need to have an email validation loop here (unless they're wanting to be anonymous) There will also be a number of consent/don't consent questions at the end
